SUMMIT COUNTY DRUG REPOSITORY PRESCRIPTION ASSISTANCE PROGRAM

UNUSED PRESCRIPTION DRUGS DONATED BY
NURSING HOMES ARE AVAILABLE AT LOW
COST TO SUMMIT COUNTY RESIDENTS IN NEED

Medications available vary from month to month based on donations and inventory. Medications do NOT include insulin products, medication requiring refrigeration or controlled substances (narcotics).

Summit County Health District is NOT collecting medications.

Who is Eligible?
• Summit County residents who have no or limited medication insurance coverage.

• 18 years of age or over

• Have no reasonable financial means to pay for the drug prescribed or be a patient of a nonprofit clinic

What is the Process?
• Complete Drug Repository Program Worksheet and the Eligibility Statement available at www.schd.org.

• If eligible, obtain written order for medication and meet with L. Stemple, RN to complete order sheet.

• Participant must provide payment to Buderer Drug Company before medication is mailed ($7.40 per prescription plus $7.00 shipping). Prescription is mailed to participant.
